ACORD Forms
ACORD forms are the standard forms used across the insurance industry. They are published by ACORD (the Association for Cooperative Operations Research and Development), a nonprofit that sets common formats so agents, insurance companies and policyholders all work from the same paperwork.
Whether you need to report a claim, prove coverage with a certificate, change a policy or apply for new coverage, there is an ACORD form for it. Below are the forms we use most. Need one? Ask and we will send it over and help you fill it out.
